I believe I found the issue and fixed it.
I think you forgot to update the .zip file. I've downloaded the file - it contains the old version 0.4.

Should now display 0.5 in the title bar.

I tested iSCSI Cake - it supports VHD and VMDK disks that are dynamically expandable but it is Trial version.
I know that the topic applies to Windows tools, but if you want something free, you can use 0.5 istgt-20150713.tar.gz on Linux - only the best to compile from sources VirtualBox 4.1.18 with the --with-vbox option:

See excellent review from Misty here about iscsi cake : did not score very well
Misty has also tested freenas here which is a freebsd variant and which may very well use istgt as well (to be checked thus).
Nowadays, the way I see it :
-on windows Starwind 5.8 (limited free version available) and iScsiconsole (freeware and opensource) are the best candidates
-not on windows, freenas (or rather XigmaNAS) has my preference and has a "embedded" ready to use version.

StarWind 5.8 unfortunately does not appear to be available anymore. If you already have an existing copy with a free license then it's worth using it - it appears less bloated than the current version 8 release and works with UEFI clients. StarWind keep changing the restrictions that come with a free license and I'm not sure which restrictions apply to v5.8 (e.g. can it mount a disk/volume as storage)....Nowadays, the way I see it :

iSCSI Console rocks. The only thing missing for my needs is a way of automating starting targets - either via the commandline or a config file. Maybe this will come in a future release. Or maybe this feature is already present, but not documented. There is a /log switch BTW.
